DESCRIPTION:  Statewide campaigns promoting the use of standard patient medication lists are growing.  When patients and their caregivers interact with health-care systems, obtaining an accurate medication history is one of the greatest challenges that confront health professionals.  Implementing a standard medication list statewide is a major initiative that can help patients with this challenge; affiliate organizations in South Carolina, Virginia, and Iowa are examples of states which have already implemented medication lists.  And, ASHP and the ASHP Foundation are spearheading a campaign to implement "My Medication List".  Participation in this webinar will assist your affiliate in promoting these forms to accurately obtain patients' medication histories and will offer hints on now to work with medical and hospital associations to implement this program.
